I have a VM using CentOS 6.x. After a Startup completes in the OnApp UI the VM doesn't come on, why?

If this VM was updated/upgraded to 6.x, try changing the grub and fstab files to point to /dev/xvde, you can do this by mounting the VMs disk within Recovery Mode.
If its a new VM, would need to check that the template is properly installed. In particular, the disk_target_device listed for the template (in the database) should have the following:
6.0 - "---\nxen: sda\nkvm: hd\n"
6.1 - "---\nxen: xvda\nkvm: hd\n"
6.2 - "---\nxen: xvda\nkvm: hd\n"
Updating the database entries and booting the problem VM should resolve this issue.
Often, what it is doing in this case is having kernel panic because it can not find / on the specified device.
